Job Responsibilities:
Provides physical/cognitive assessment services to clients who have impairments, functional limitations, disabilities or changes in physical function and health status resulting from injury, disease or other causes.  Provide physical/cognitive assessment to determine ability to ride public bus/LantaVan.
A. Delivers quality care to include, but not be limited to:Reviewing all pertinent intake information on the client prior to assessing abilities; conducts a pre-assessment review of intake information with the client to clarify information to assure a throughout understanding of all pertinent information on the individual; conducts a thorough physical/cognitive assessment of client abilities and accurately noting results as testing is administered including appropriate behaviors/attitudes as part of the assessment.
B. Records and documents services in a timely and accurate manner to include, but not be limited to:Additions of written comments to initial intake information that clarifies, adds, corrects information based on pre-assessment review with client; completion of assessment scoring sheet with performance results at the time of the assessment; completes behavior/attitude observations and makes a determination based on results of client’s eligibility public bus/LantaVan services; provides written information to Lanta within two working days of the completion of the assessment; completes daily time sheet to record billable hours to Lanta; completes service database, including assessments completed and disposition/outcome of assessment on a daily basis.
C. Provides outstanding customer service to include, but not be limited to:Greeting clients and families and explaining the process for the completion of the physical/cognitive assessment; answering client questions regarding the process to best of ability and provides resources (Goodwill staff/Lanta staff) for other information; indicates to client how results of the assessment will be communicated to them and identify the timeframe for that communication.
D. Participates in continuing education and reviews professional publications on topics relevant to disability, transportation issues for persons with disabilities, physical/cognitive assessments and other topics related to current job skills.  Learns additional skills to enhance ability to serve as a back-up to other staff to include Lanta and job coaching assistance.
E. Performs all duties in a safe manner and ensures that clients adhere to all safety practices and guidelines.
F. Communicates progress, concerns issues with the Lanta Program Manager and the Director of Vocational Services Allentown.
Job Qualifications:
BA or BS in Parks and Recreation or Leisure studies with an emphasis in Therapeutic Recreation
Certification as a Certified Therapeutic Recreation Therapist and maintains certification
Bachelor’s degree in Exercise Science or Kinesiology
Certified Occupational Therapist Assistant Physical Therapist Assistant license Clinical experience in physical rehabilitation preferred
Ability to multi-task and prioritize job responsibilities
Ability to recognize critical situation and respond effectively
Good communication skills – verbal and written
CPR Certification
Valid driver’s license and reliable vehicle
Must complete and successfully pass State background check, Child Abuse Clearance and FBI Clearance
